A new Hospital at Home service for older adults with mental illness is set to launch across Coventry and Warwickshire this summer.
The service will offer hospital-level care in patients' homes to reduce unnecessary admissions and support recovery in the community.
Developed by the Coventry and Warwickshire Partnership NHS Trust, the service is designed to provide personalised, multidisciplinary care for older people experiencing mental illness who would otherwise require inpatient treatment.
Patients will receive tailored support at home, with up to four visits per day depending on their clinical needs.
The initiative aligns with the NHS's ambition to shift more care from hospitals into community settings, helping people remain in familiar surroundings while maintaining links with family and carers.
The trust said the service is due to launch in August, and that inpatient care will continue to be available where clinically appropriate.
The service has been shaped through engagement with patients and carers, alongside analysis of local clinical data, which identified opportunities to prevent avoidable hospital admissions.
Feedback from service users has informed the design of the new care model to ensure it is responsive and tailored to local needs.
The Trust will monitor the service's performance through measures including patient outcomes, readmission rates, hospital length of stay, and patient and carer feedback to support ongoing service improvement.
Alongside providing clinical care, the service will work with local authorities and voluntary organisations to connect patients with wider community support.
Where appropriate, this includes helping to address issues such as loneliness and social isolation while enabling more people to receive treatment safely at home.
