Genuine transformation extends far beyond simply implementing new technologies within the confines of individual hospitals; it necessitates the construction of a fully connected ecosystem that benefits the entire healthcare system, from primary care to specialist services.

The Humber Health Partnership is actively working towards this vision by focusing on expanding its virtual ward capabilities.

This initiative aims to prevent unnecessary hospital admissions by providing patients with remote monitoring and care in the comfort of their own homes.

This not only improves patient experience but also frees up valuable hospital beds and improves capacity management across the entire system.

By leveraging technology to deliver care closer to home, the Partnership is creating a more sustainable and patient-centred healthcare model.

The Partnership’s approach to digital transformation is firmly grounded in two key principles: a focus on making foundational improvements while simultaneously planning for strategic, long-term change, and a commitment to delivering continuous improvement rather than striving for an unattainable state of perfection.

This pragmatic approach recognises that digital transformation is an ongoing journey, not a destination.

This philosophy is clearly exemplified by our Electronic Patient Record (EPR) programme. While actively working towards procuring a new, state-of-the-art EPR system, the Partnership is concurrently implementing practical improvements that deliver immediate and tangible benefits to both patients and staff.

This dual approach ensures that momentum is maintained and that patients and staff experience positive changes in the present, while also progressing towards larger strategic goals.

The Partnership is also making significant investments in new data warehouse infrastructure to enhance its analytical capabilities.

This investment will empower better informed decision-making at all levels of the organisation and will provide crucial support for broader digital transformation initiatives.
