Chelsea and Westminster Hospital NHS Foundation Trust has launched a new digital postnatal discharge system, powered by Lumeon technology.
The innovative software will improve the patient experience, increase efficiency, and reduce costs at the hospital.
Funded by CW+, the official charity of the trust, Lumeon’s Care Pathway Management solution digitises the postnatal pathway.
All activities and tasks are orchestrated and automated in real time, with the ability to get a good overview of the care being delivered.
Digital dashboards allow multi-disciplinary team members to quickly see what they need to do next, and by when, for each patient to meet their discharge slot.
Staff are then able to communicate more effectively with new mums, and provide a more-holistic overview relating to them and their baby as regards getting them ready for home.
