The Practice Group showcases improvements to patient care
Innovative new approaches are in the spotlight at the Best Practice Show
One of the largest nationwide independent NHS providers of primary and community based care, The Practice Group, will be highlighting improvements in patient care through innovative breakthrough technology, staffing models and data analytics on Stand F74 at the Best Practice Show on 19-20 October at the NEC in Birmingham.
Recognising that different areas have diverse requirements, The Practice Group has created models around crucial elements, including clinical patient pathways, operational and staffing departments.
The enhanced services include:
- Creating designated care coordinators within each surgery, to provide holistic care to patients by signposting them to local support groups, exercise classes and social care programmes dependent on their needs;
- Addressing capacity issues within Shropshire Clinical Commissioning Group’s (CCG) Hospital Eye department by launching a Community Ophthalmology Service in the area. This is helping to move patients from referral to treated within the specified 18 weeks;
- Developing a new data analytics solution enabling individual surgeries to identify trends and issues occurring within their areas so that action can be taken on aspects of patient care where improvements are possible.
'We are continually working with both the NHS and our practices by developing and deploying new initiatives in primary and community care, all with the aim of delivering the best possible care to patients', said Allan Johnson, CEO of The Practice Group.
