Patients Know Best and HealthUnlocked provide GPs with social prescribing platform to support clinical treatment


Patients Know Best and HealthUnlocked have signed a partnership agreement in a bid to improve social wellbeing for patients.

The partnership which will see HealthUnlocked’s eSocial Prescription capability integrated with PKB’s award-winning digital platform, making it easier for healthcare professionals to offer their patients more-holistic, personalised care plans in the form of a social prescription that aims to complement clinical treatments.

eSocial Prescriptions generated by HealthUnlocked, via the GP’s or specialists clinical systems, are sent to patients as an email. This is mirrored in their PKB personal health record, where they can also find local resources and unique, condition-specific online communities.

Validated research has proven that when patients join these communities, they visit their GP or local emergency department much less, and increase their patient activation measure.

This increased capability supports the national direction of care from the Department of Health and Social Care, which is working on a series of social prescribing programmes to combat over-medicalising people and helping the NHS to save money.
