Improved patient care with Physitrack

Connect Health, is rolling out Physitrack, a new web and iOS-based exercise app that allows clinicians to engage with patients around high-quality exercises and patient information.

Patients can instantly access information via the PhysiApp patient portal or downloadable app, where they can engage visually with a clinician, watch video demonstrations of their prescribed exercises, and record compliance, enhancing patient outcomes and improving choice.

The investment in Physitrack ties in with Connect’s digital strategy, which includes linking to predictive analysis on how to build up care plans, video consultations, live exercise demos and AI in order to make systems more efficient and effective.

Developments in digital health will be groundbreaking for patients and for the MSK industry as a whole.

Some of the benefits of Physitrack are:

  • A wealth of high-quality videos accurately display how exercises should be performed
  • Patients can record compliance including how much pain they felt, how easy it was, and what they have been able to complete
  • Ability for clinicians to select relevant exercises and outcome measures and build bespoke templates
  • Access to Connect standard exercise templates, patient information leaflets and other useful information, both pictures and videos
  • Reporting capabilities, allowing the storage and analysis of the data inputted in order to provide better services

Jo Harrison, a senior occupational physiotherapist, said: “The bespoke function is really useful and allows us to tailor the exercise programmes to our individual patients’ needs.

“If you know the specific exercises you want to use you can simply search for it by name, for example ‘knee rolls’, then adjust and change as required.

“The ready-made templates are also a really-useful tool to make it quick and easy for clinicians to create programmes, especially for common conditions, knowing they are based on the most-up-to-date research and evidence-based practice.”

Physitrack is being implementing across all Connect Health contracts, both NHS and occupational health, to enhance the patient journey, improving access to professionals and bespoke video exercise programmes.
